Care Management: What it is and how it works

Navigating the health care system can be confusing, particularly if you are facing a chronic or complex health condition. Care Management can help members improve their self-management skills, get them connected to key resources in the community and coordinate care when multiple providers need to be involved in the treatment plan. Key Care Management staff are called Case Managers. An easy way to understand Care Management is to consider the words themselves: We CARE and we MANAGE (members’ health care and industry health care dollars). Case Managers

Case managers are registered nurses or other health care professionals who work in a variety of settings. These include long-term care, mental health care, hospitals and state organizations. Case managers help members develop plans and goals for their care. They provide explanations for such questions as “What’s next?” They oversee treatment and assist in the transition from hospital to home or other settings. They help get members to appointments and encourage them to follow doctors’ orders.

In short, case managers: 
• Assess the situation 
• Determine the needs 
• Evaluate the options 
• Organize the resources 
• Promote quality, cost-effective outcomes for the member.

 

Health Risk Assessment Survey

Every year, members receive the Health Risk Assessment Survey (HRA) in their member enrollment kit. The HRA asks questions about your medical history, lifestyle, medications and more. Once we have collected this information, Soundpath Health shares the completed survey with your Primary Care Provider to ensure that your PCP is informed. In addition, Soundpath Health relies on your survey responses to continually improve our programs and services for you.

If you have not completed a Health Risk Assessment Survey in the last 12 months please click here to print out and mail in your completed survey. The results are confidential and will only be shared with your personal physician. 

 

 

 

National Surveys 

Soundpath Health collects standardized information on clinical and service performance measures to assess how we are doing in areas important to our Member's care. We use three standard national survey processes: Healthcare Effectiveness Data and Information Set (HEDIS), Consumer Assessment Health Plan Services (CAHPS) and Health Outcomes Survey (HOS). These help us determine how we are doing, how we compare to other plans and guide us in continually improving our performance. Soundpath Health will share information with you on important survey results through our website and other communication venues.

 

Member Resources & Patient Safety

There is a wealth of information and resources publically available on a wide variety of health topics. Some of this information is reliable and some is questionable. When it comes to your health and well-being your Physician's Office should always be consulted.
