Our programs are centered around you, the member, and our goal is to provide you with the wellness information you need to maintain your good health and stay on track with your health care needs. 2011 Quality Initiatives

In 2011, Soundpath Health is focuses on four wellness initiatives. 

Falls Prevention
Did you know that every year one third of those over 65 and living at home will fall? Do you know what you can do to reduce your risk of falling? Care Management

Care Management is a comprehensive method of client assessment which is designed to identify client vulnerabilities, needs and goals. The goal is to provide client advocacy, a system for coordinating client services, a systematic approach to evaluation and most importantly, to address your needs foremost.

You and your loved ones may benefit from care coordination if you suffer from an illness that requires daily services or care. If your health problems require coordination of multiple services Soundpath Health will help to ensure that you receive appropriate care in the appropriate setting.

Health Risk Assessment Survey

Every year, members receive the Health Risk Assessment Survey (HRA) in their member enrollment kit. The HRA asks questions about your medical history, lifestyle, medications and more. Once we have collected this information, Soundpath Health shares the completed survey with your Primary Care Provider to ensure that your PCP is informed. In addition, Soundpath Health relies on your survey responses to continually improve our programs and services for you.

If you have not completed a Health Risk Assessment Survey in the last 12 months please click here to print out and mail in your completed survey. The results are confidential and will only be shared with your personal physician. 

National Surveys 

Soundpath Health collects standardized information on clinical and service performance measures to assess how we are doing in areas important to our Member's care. We use three standard national survey processes: Healthcare Effectiveness Data and Information Set (HEDIS), Consumer Assessment Health Plan Services (CAHPS) and Health Outcomes Survey (HOS). These help us determine how we are doing, how we compare to other plans and guide us in continually improving our performance. Soundpath Health will share information with you on important survey results through our website and other communication venues.

Member Resources & Patient Safety

There is a wealth of information and resources publically available on a wide variety of health topics. Some of this information is reliable and some is questionable. When it comes to your health and well-being your Physician's Office should always be consulted.

Soundpath Health has established a link to connect you with current and reliable Patient and Consumer information. The resources here are intended to inform you as well as guide your communications with your Physician and Health Care Team.
