Medication Therapy Management Program

Soundpath Health's Medication Therapy Management Program (MTMP) identifies beneficiaries who are at risk for adverse medical or drug events. The MTMP triggers actionable interventions to physicians to review therapy, and encourages compliance with appropriate dosing, length of therapy and national, evidence-based clinical guidelines.

Beneficiaries who meet the following criteria are automatically enrolled in the MTMP:

  • Have two or more targeted chronic diseases, such as:
    • Hypertension
    • Asthma
    • Behavioral health (Depression)
    • Diabetes
    • Dyslipidemia
    • Chronic Heart Failure
    • Chronic Obstructive Pulmonary Disease (COPD)
  • Are taking at least six covered Part D drugs
  • Are likely to incur annual costs for covered Part D drugs in excess of $3,000 for 2011

Beneficiaries participating in this program benefit from receiving the following education regarding common health-related issues:

  • Medication action plan and educational materials
  • One-on-one personalized telephone counseling with a pharmacist
